The Four Steps

Recognising the Structural Gap (PATCG)

The roadmap begins with identifying the Post-Acute Trauma Continuity Gap (PATCG), the structural discontinuity that frequently emerges between acute discharge and sustained recovery support following Road Traffic Accidents (RTAs). Without recognition of this gap:

  • Continuity remains assumed rather than structured
  • Responsibility diffuses across services
  • Governance visibility weakens over time

The roadmap first makes the gap visible.

Applying the Structural Response (PATS Model™)

Once the gap is recognised, the Post-Acute Trauma Support (PATS) Model™ provides the structured response. It is a clinically aligned continuity framework designed to restore structured support within post-acute pathways. It connects:

  • Acute care
  • Rehabilitation services
  • Long-term recovery oversight

It does not replace existing services. It strengthens how they connect.

Translating Structure into Governance Alignment

Recognition and model application must sit within governance oversight. The roadmap maps TPS architecture against:

  • CQC domains (Safe, Effective, Caring, Responsive, Well-Led)
  • NICE-aligned trauma recovery standards
  • Documentation and audit frameworks
  • Clinical governance responsibilities

This supports structural continuity that is not only clinically aligned, but regulatorily visible.

Internal Governance Reflection

The final component of Pathway 1 is a compliance self-check. It allows organisations to reflect on:

  • Where post-discharge continuity currently sits
  • Whether governance visibility extends into long-term recovery
  • How referral workflows maintain responsibility
  • Where structural reinforcement may be needed

This is not a submission. It is an internal reflection mechanism that supports informed progression.
